Essential Requirements for Registered and Enrolled Nurses

  • Experience preferably in Aged Care
  • A clear understanding of  Accreditation Standards
  • Possession of a current AHPRA Nursing Registration
  • A team player with excellent interpersonal and communication skills
  • A desire to uphold high standards of person centred care and work
  • Ability to work autonomously

Occupational Therapist – PD Occupational Therapist Community MAY 2022 v2

Due to rapid growth, Corumbene Community Services is seeking an Occupational Therapist. The position is part time & fixed term for 12 months.

Qualifications & experience

  • Highly developed organisational and time management skills including the ability to prioritise work under pressure and to work to timelines
  • Highly developed skills in conducting consumer assessments in their home and developing reports with an understanding of the Aged Care Standards and funding guidelines.
  • High level interpersonal and communication skills, including the ability to communicate effectively with staff, consumers and their representatives, and other external stakeholders.
  • The ability to work as part of a team, the capacity to be self-directed and the willingness to learn.

Tasks & responsibilities

  • Occupational Therapy assessment of consumers referred through My Aged Care
  • Report preparation and follow up of completed work
  • Work with Community Services Case Managers and Care Coordinator to promote consistency in and continuity of consumer centred care and services
  • Contribution to continuous improvement and quality assurance activities

Community Programs Coordinator  – PD Programs Coordinator May 2022 v2

This position is for a 9 day fortnight.

As a Community Programs Coordinator you will be coordinating consumers receiving funded support, including CHSP, DVA, Private and Brokered. On boarding, goal setting and care planning, monitoring progress and reviewing services in accordance with funding guidelines whilst providing continuity and consumer centred care.

Desirable Requirements

  1. Experience in care coordination, case management

Required Attributes and Qualifications

  5. A current Driver’s Licence

Assisting in maintaining and enhancing the physical, emotional and social wellbeing and independence of older people, their carers and families who are assessed as needing support to remain in their own homes.
