CORUMBENE CARE HOSTS EDUCATION EVENING AT MONA

As announced by Primary Health Tasmania last December, Corumbene Care is responsible for the provision of services to support people living with heart disease, lung disease, musculoskeletal conditions and diabetes, who now have better access to help them stay as healthy as possible, while staying at home.

Last Monday evening, Corumbene Care hosted a dinner and panel Q&A for twenty-five guests, many of whom are health professionals responsible for supporting people living with chronic health conditions. The partnership between health professionals and Corumbene Care is a crucial element in the delivery of care.

The services are part of a new Australian Government-funded program focused on improving the health and wellbeing of people living with chronic conditions in rural areas.

Corumbene Care CEO Damien Jacobs, says the rural primary health services add to an already comprehensive range of products available through Corumbene Help at Home, a service which continues to grow and expand in response to community needs and emerging health priorities.

“The service targets people in the areas of the Derwent Valley, Central Highlands and Southern Midlands who are living with cardiovascular disease, chronic lung conditions or musculoskeletal disorders,” Mr Jacobs said.

“Corumbene Help at Home will also be working alongside Diabetes Tasmania to improve the health of people with diabetes. These services will include providing allied health, care coordination and support for people to self – manage their condition using the latest in technology, including tele- monitoring”.

The services will be run in collaboration with the patient’s general practitioner and health care team to ensure that they can self-manage their condition to improve their quality of life.

“It also ensures patients can better navigate the health care system which allows them to remain as independent as possible within their own community,” Mr Jacobs said.
